Warum Verhütung eigentlich nicht nur Frauensache sein müsste und welche Konzepte es zur Verhütung für das männliche Geschlecht gibt.
Etwa 60 Jahre nach Einführung der „Pille“ bleibt der Feminismus auch weiterhin ein wichtiger Motor in unserer heutigen, ach so aufgeklärten Gesellschaft. Und wenn man schon dabei ist, sich über die Senkung der Mehrwertsteuer auf Monatshygieneprodukte zu freuen, könnte man(n) sich einmal durch den Kopf gehen lassen, wie es denn hier so mit der Geschlechtergleichstellung bei der Verhütung aussieht. Warum dürfen sich eigentlich in letzter Konsequenz meist die Frauen darum kümmern, dass aus Spaß kein „Ernstl“ wird. Mir ist das in meinen Praxisräumen schon immer sehr zuwider, wenn mal wieder eine Dame geduldig ansitzt, damit sie nach oftmals kurzer, aber keinesfalls gerechtfertigter Wartezeit eine Unterschrift auf ihrem Pillenrezept erhält. Für dieses kommt sie dann in der Apotheke ab dem 22. Lebensjahr komplett selbst auf und erkauft sich damit noch gut und gerne so „lebensbereichernde“ Nebenwirkungen wie ein erhöhtes Thromboserisiko, Akne, Beinödeme, Gewichtszunahme, Kopfschmerzen, Libidoverlust, Stimmungsschwankungen und so weiter und so fort.
Doch welche Konzepte existieren für den Mann neben den bekannten Verdächtigen und wie schaut es aus mit der Sicherheit? Die Zuverlässigkeit von Verhütungsmitteln wird übrigens mit dem sogenannten Pearl-Index gemessen, benannt nach dem amerikanischen Biologen Raymond Pearl. Er beschreibt, zu wie vielen Schwangerschaften es kommt, wenn 100 sexuell aktive Frauen ein Jahr lang mit einer festgelegten Methode verhüten.
Kondome
Mit einem Pearl-Index von 2 bei richtiger Anwendung sicherste Verhütungsmethode und am breitesten verfügbar. Sie sind in aller Regel aus Latex hergestellt, es existieren aber auch Alternativen für Allergiker. Zudem bieten Sie als einzige Variante der bekannten Verhütungsmethoden einen zuverlässigen Schutz gegen sexuell übertragbare Infektionen. Gerade aus letzter Sicht sind Zumutbarkeit und Relevanz bei allen geplanten und ungeplanten Vorhaben im Zweifel keinesfalls zu diskutieren.
Hormonelle Methode/die „Pille für den Mann“
Ähnlich wie bei den weiblichen Artgenossen funktioniert die Produktion der für die Fortpflanzung wichtigen Zellen über die Pegel bestimmter Hormone, wobei für die ausreichende Spermienproduktion eine ausreichend hohe Testosteronkonzentration im Hoden erforderlich ist. Diese wird von der Hirnanhangsdrüse, der sogenannten Hypophyse hochgehalten. Sinken die Spiegel, setzt die Hypophyse Hormone frei, die wiederum weitere Hormonspiegel anfeuern, die ihrerseits in bestimmten Zellen die Produktion von Testosteron ankurbeln. Gaukelt man nun der Hypophyse durch die künstliche Zufuhr von Testosteron hohe Spiegel vor, so setzt sie weniger der zur Testosteronproduktion nötigen Hormone frei und die Spermienproduktion im Hoden lässt aufgrund fehlenden Treibstoffes auf natürliche Weise effektiv nach. Mit deutlich reduzierter Spermienzahl im Ejakulat wird eine erfolgreiche Befruchtung ähnlich unwahrscheinlich, wie wenn die Partnerin die Pille einnehmen würde. Ähnlich wie bei der „Pille“ wurden zunächst Mono-Präparate – also die reine Testosterongabe – in Form einer Spritzengabe getestet; in Tablettenform ist das Hormon leider nicht täglich verabreichbar. Nach vermehrten Nebenwirkungen im weiteren verfeinert als Implantat und durch die Hinzunahme weiterer Hormone mit abgemildertem Nebenwirkungsprofil konnte sich kein Präparat trotz angeblich hoher Toleranz vonseiten der Probanden durchsetzen und zur Marktreife durchdringen. Als Hinderungsgründe wurden die im Vergleich zur Pille leicht erhöhten Kosten und die weiterhin beschriebenen Nebenwirkungen angegeben: Akne, Gewichtszunahme, Libidoverlust, Verlust an Muskelmasse, Stimmungsschwankungen und so weiter. Für die einen also tolerabel, für die anderen unzumutbar?
Nicht-hormonelle Methoden
Hier gibt es zahlreiche Ansätze, die alle noch nicht wirklich ausgereift sind. Es existieren bereits Wirkstoffe, die die Bildung der Spermien durch Hinderung der Verstoffwechselung von Vitamin A vermindern, das für die Bildung der Spermien in den Genitalorganen essenziell ist. Ein weiterer Angriffspunkt ist das Spermium selbst. In Tierversuchen haben sich bereits einige Präparate als zeitweise („nicht“) zielführend erwiesen, indem sie die Beweglichkeit der Spermien herabgesetzt haben. Die versammelten Keimzellen erreichten so auch mit vereinten Kräften die begehrte Eizelle bzw. waren zu träge, um sich dort gezielt einzunisten. Die beschriebenen Ansätze sind allzeit reversibel, also wieder rückgängig machbar und mit wenigen Nebenwirkungen behaftet. Mal sehen, ob dies Argumente sind für die erfolgreiche Entwicklung oder ob die weitere Entwicklung auch aufgrund „mangelnder Attraktivität“ unter der Hand wieder eingestellt wird.
Teilweiser Verschluss der Samenleiter
Mit Injektion eines Polymergels werden die Samenleiter im Rahmen der RISUG® (Reversible Inhibition of Sperm Under Guidance) nach zehn Tagen für die Spermien nicht ohne Beschädigung passierbar. Mittels Verfeinerung des Gels durch Kupfer- und Eisenoxidpartikel (SMA-Gel) wurde die Spermien-schädigende Wirkung noch erhöht, noch dazu ist die Lage und Form durch Magneteinwirkung veränderbar. Die beschädigten Keimzellen sind zu einer Befruchtung nicht mehr fähig. Der teilweise Verschluss kann auf Wunsch durch Injektion eines Lösungsmittels, das das Gel zur Auflösung bringt, wieder rückgängig gemacht werden und hält ohne Verwendung von Lösungsmittel für zehn Jahre. In Indien befindet sich das RISUG-Verfahren derzeit vor der Marktzulassung, in einigen anderen Ländern ist das Prinzip patentiert und die Markteinführung wird wie beim „Vasalgel“ in den USA angestrebt. Offen bleibt – und das ist der große Kritikpunkt – ob der teilweise Verschluss wirklich so sicher und einfach bzw. überhaupt rückgängig gemacht werden kann. Falls nicht, wäre der Unterschied zum letzten Schritt auch nicht mehr groß.
Vasektomie
Mit einem Pearl-Index von 0,2 ist dieser kleine Eingriff noch sicherer als Kondome. Er ist aber nur bei bereits abgeschlossener Familienplanung zu empfehlen, da er nicht so einfach, sondern nur durch einen weiteren chirurgischen Eingriff und nach langer Zeit sogar gar nicht mehr rückgängig gemacht werden kann. Durch verschiedene OP-Techniken werden beide Samenleiter mit einem Schnitt durchtrennt und die Enden mittels Verödung oder kleinen Clips verschlossen und getrennt fixiert. Die Spermien können so nicht mehr ins Ejakulat gelangen, stauen sich aber nicht wie oft befürchtet an, sondern werden vom Körper unbemerkt einfach abgebaut. Die Komplikationen halten sich in Grenzen: 14 Tage sollte im Anschluss kein Sport getrieben werden, kleine Hämatome und Wundinfektionen sowie Hodenschmerzen können auftreten, sind aber selten.
